As a Clinical Liaison, you will play a vital role in connecting patients, families, referral partners, and care teams with the services CORALVITA provides. You’ll serve as the bridge between our clinical excellence and the communities we support—educating partners, assessing patient suitability, coordinating transitions of care, and ensuring that every individual entering our facility is set up for success.

This role requires strong clinical knowledge, relationship-building skills, and the ability to communicate clearly and compassionately across diverse audiences.

Responsibilities

  • Build and maintain relationships with hospitals, physicians, case managers, and other referral sources to represent CORALVITA’s services.
  • Conduct on-site and remote clinical assessments to determine patient eligibility and appropriateness for admission.
  • Serve as the primary point of contact for referral partners regarding patient status, documentation, and required clinical information.
  • Coordinate with nursing, therapy, and admissions teams to ensure seamless and timely transitions into care.
  • Educate referral partners on CORALVITA’s programs, capabilities, and clinical pathways through outreach, presentations, and ongoing communication.
  • Document all referral activity, assessments, and communication in the electronic health record (EHR) and CRM systems.
  • Support patient and family understanding of the care transition, addressing concerns, answering questions, and ensuring clarity of expectations.
  • Collaborate with internal leadership to identify outreach opportunities, improve community relationships, and expand market presence.
  • Participate in interdisciplinary team meetings to communicate referral trends, patient needs, and operational improvements.
  • Uphold CORALVITA’s values of empathy, excellence, and clinical integrity in all interactions.

Qualifications

  • Education: Licensed RN, RT, PT/OT, LPN/LVN, or other clinically licensed background preferred; equivalent clinical experience considered.
  • Certification: BLS required or obtained within 30 days of hire.
  • Experience: Previous experience in admissions, clinical liaison work, case management, or discharge planning strongly preferred.
  • Strong clinical judgment and ability to conduct thorough patient assessments.
  • Excellent communication skills with the ability to build trust and rapport quickly.
  • Knowledge of post-acute care, long-term acute care hospitals, or complex-care environments is highly beneficial.
  • Proficiency in documenting assessments, coordinating care, and working within EHR/CRM systems.
  • Ability to travel locally for partner outreach and on-site evaluations.
