What to Bring

When arriving for admission, it’s important to bring identification and insurance documents, as well as any advance directives or power of attorney paperwork. Having a clear record of your loved one’s current medications and dosages is also essential, since this allows our team to continue care seamlessly. While the hospital provides the medical basics, personal comfort items — such as non-skid socks, glasses or hearing aids, and a few familiar belongings — can make the patient feel more at home. Families often find it helpful to carry a small notebook to keep track of updates, care team names, and questions that come up along the way.

The Rhythm of Recovery

Admission Day

You’ll meet with our admissions staff and be guided through orientation. The clinical team will review the patient’s medical records, complete an assessment, and finalize the treatment plan. Once your loved one is settled into their room, the nursing staff will explain daily routines and answer any immediate questions.

Using Resources

At CORALVITA, our team is here for you too. Case managers, social workers, and chaplains can provide guidance, connect you with resources, and support your emotional well-being. Ask a trusted friend or family member to share updates with others, coordinate meals, or help with errands. You don’t have to carry this alone.
