Our Case Management team plays a vital role in helping patients transition safely, confidently, and with dignity — and we’re looking for a dedicated Case Management Assistant to support this mission.

Responsibilities

The Case Management Assistant provides clerical and administrative support to the Case Management team, helping facilitate care coordination, discharge planning, and utilization review. This role supports day-to-day case management operations including:

  • Tracking concurrent review dates and obtaining updated clinical information
  • Preparing, organizing, and faxing managed care forms
  • Verifying receipt of authorizations and documenting updates
  • Assisting with post-discharge referrals, follow-up calls, and care coordination
  • Organizing discharge packets for Skilled Nursing Facilities, Subacute care, and Acute Rehab
  • Helping patients secure post-discharge transportation and durable medical equipment
  • Coordinating appointments with primary and specialty care providers

This position ensures that all documentation is accurate, timely, and complete within the HMS financial system and supports the Case Managers in delivering safe, high-quality patient transitions.

Care Coordination Support — 40%

  • Attend daily Case Management huddles and gather task assignments
  • Assist Case Managers with discharge arrangements for current and future discharges
  • Deliver Important Messages to Medicare patients once trained
  • Arrange referrals and follow-up calls with post-acute providers
  • Coordinate services such as home health, dialysis, outpatient infusion, and DME
  • Prepare complete discharge packets for transitions to SNF, Subacute, and Acute Rehab
  • Assist with scheduling post-discharge physician visits

Reimbursement Support — 40%

  • Assist patients with Medicaid applications as requested
  • Review concurrent stay lists, prepare authorizations, and send documentation to payors
  • Obtain clinical information from the EMR or Managed Care Update Form
  • Fax documentation and follow up with payors for authorization confirmations
  • Accurately document all continued-stay activity in HMS
  • Ensure all correspondence from payors is scanned and recorded

Administrative Duties — 15%

  • Answer phones, take messages, and coordinate meeting logistics
  • Prepare and distribute materials, invitations, and minutes
  • Participate in data collection and tracking projects
  • Organize departmental mail and office supplies

Compliance — 5%

  • Maintain full adherence to hospital and Case Management policies
  • Protect patient confidentiality at all times
  • Support compliance with federal and state regulations
  • Report suspected violations of policies, regulations, or confidentiality

Minimum Qualifications

  • High school diploma or equivalent
  • Knowledge of medical terminology
  • 1 year of experience as an Administrative Assistant or similar role
  • Previous experience in a healthcare or insurance environment
  • Strong computer, data-gathering, and documentation skills

Preferred Qualifications

  • One year of college
  • Acute care hospital experience
  • Experience with Third Party Payors
  • Knowledge of reimbursement programs, discharge planning, and case management
  • Familiarity with federal/state regulatory requirements and hospital procedures
  • Strong organizational, prioritization, and analytical skills
  • Ability to work under deadlines and independently when needed
  • Professional communication and interpersonal skills
